A revision is a new manufacturing basis; it does not silently replace the basis already used for completed work.
Every HOLD identifies affected work targets and has one named decision owner.
Rework is connected to the original issue and leaves the before-and-after evidence intact.
Release is derived from approved basis, resolved issues and required inspection evidence—not from a manually colored status cell.
